The order process starts with this request form. Whether you know exactly what you want or you just have a general idea and are looking for direction, this form will guide you through the choices of:

  • Apparel Styles
  • Colors
  • Customization

The steps of the design process are:

  • Fill out this Design Request Form and pay the non-refundable $50 deposit.
  • Revise and approve the design.
  • Fill out the Custom Order Sheet. (This is available once the design is approved)
  • Pay the remaining order balance.
  • Receive the order within 10-15 business days.
Disclaimer: Please note at the end of this design request you will be asked to provide a $50 deposit before submitting. Although this deposit is nonrefundable, the amount is credited toward your final order total. You can pay the deposit via Zelle, Venmo, Apple Pay, or Cash App on the final page of this request form.

Reminder - The steps of the design process are:

  • Fill out this Design Request Form and pay the nonrefundable $50 deposit.
